Root Resection: An Alternative Treatment Option to Extraction

چکیده
Root resection is the process by which one or more of the roots of a tooth are removed at the level of the furcation while leaving the crown and remaining roots in function [1]. Indications for root resection therapy include: teeth with periodontal problems, endodontic problems, root fractures, and prosthetic problems [2]. Vertical root fracture (VRF) is an untoward complication to root canal therapy that often calls for tooth extraction. Aetiological factors for VRF include loss of substance due to restorative and endodontic therapy and stress factors associated with root canal debridement and filling [3]. Root canal preparation causes stress in dentin and may initiate dentinal cracks, which can extend to complete fractures under masticatory forces [4]. VRF usually originates from the apical end of the root and propagates coronally or depending on the nature of stresses, originate from the cervical portion of the root with extension in an apical direction [4]. Usually, patients of middle and older age are affected. An overall prevalence of 3% to 5% has been reported in literature [5]. The clinical presentation of VRF is extremely variable and may include fistula and abscess formation, development of a deep, narrow and isolated pocket, radiographically visible bone resorption along the lateral surface of the root and depending the on the angle of the X-ray cone beam, radiographically visible fracture line [6].

Root Amputation: A Prudent Alternative to Tooth Extraction
Compromised teeth often pose a significant challenge to the clinician, complicating the treatment plan and compromising long term prognosis. Upper molar teeth showing periodontal attachment loss, caries or fractures involving the furcation area are prime examples of such challenges. Treatment option for affected teeth can be roughly divided into maintenance of the tooth through root resection t...

Vertical root fractures (VRF) in endodontically treated teeth have long been reported and pose diagnostic difficulties. A hemisection/root resection procedures removes the fractured fragments completely, and retains a portion of the compromized tooth offers a predictable treatment option. The key to this rests in ideal case selection involving balancing all indications and contraindications. Th...

Instead of extraction this case report presents an alternative treatment option for a maxillary incisor with a vertical root fracture (VRF) causing pain in a 78-year-old patient. After retreatment of the existing root canal filling the tooth was stabilized with a dentine adhesive and a composite restoration. Then the tooth was extracted, the VRF gap enlarged with a small diamond bur and the exi...

This case series aims to comprehensively introduce intentional replantation with a focus on its indications and case selection in endodontics. In all represented cases, calcium enriched mixture (CEM) cement is used for root-end filling. This case series demonstrates twenty cases of IR and extraoral root-end resection and filling with CEM cement. All the selected teeth had a failed endodontic tr...
